I guarantee you that DeForest Kelly never incorporated into his characterization over the years any memories of guilt over euthanizing... whoever that was that he euthanized in the 5th movie, since that had never been written for him until then. Although certainly not a stretch, especially for a medical character's backstory.

His father. The upshot was part of the argument against euthanization, in that a cure can be found at any point, which is exactly what happened; Bones found the cure.

The other example (that I'm sure we discussed at some point in some context) was from college, when I was in a production of Hamlet, and the director was telling us that we were in a vaguely Renaissance/Elizabethan time setting, technically Denmark but by way of England, with no specifics. So based on the locale (and references to various Catholic practices in the script) it was logical that everyone was Catholic, even though it mattered very little to the story - this was just something the cast could silently incorpopate into their backstories. But, he added, if anyone wanted to be a closet Lutheran, that was probably OK too.
