I punt for the former. Minas Ithil was very heavily beseiged, all the might of Gondor could not lift the seige in two years. No doubt the defenders of Minas Ithil kept up palantir communication (p-mail ?) with the King in Minas Anor. When the Nazgul entered the city the defenders may have tried to destroy or hide the the Stone. Obviously to no avail.
